What is LV myocardial performance score (MPS)?

MPS combines the API and conventional-CPO measures of efficiency and power into an emerging LV performance score. It was developed using physiological modeling and studied in clinical HF cohorts. It is different from the RV myocardial performance score and the echocardiographic Tei index.

How is it calculated?

API × conventional CPO / 2

API is systemic pulse pressure divided by mean PAWP; conventional CPO is CO × MAP / 451 in watts. Use API × CPO / 2 for this LV score. Do not substitute RAP-adjusted CPO or an RV-power measure.

Result units: score.

Clinical interpretation

MPS below 0.5 was associated with transplant-waitlist removal for death or deterioration in a registry cohort. That prognostic association does not establish a universal threshold for ventricular uncoupling, a normal range, or a treatment target. Further validation is needed across clinical settings.

Clinical considerations

The direct-input mode requires API and conventional CPO from the same hemodynamic snapshot. The measurement mode derives both from systemic blood pressure, mean PAWP, cardiac output, and MAP.

Errors in the wedge, flow, or blood pressure propagate through both component measures. A score above a study reference cannot establish adequate perfusion or justify withholding treatment.
