What is RVSP?
RVSP means right ventricular systolic pressure. Echocardiography estimates it by adding the RV–RA systolic pressure gradient to estimated right atrial pressure.
How is it calculated?
RVSP (mmHg) = 4 × [peak TR velocity (m/s)]² + RAP (mmHg).
Clinical interpretation
The 2025 ASE right-heart guideline generally considers RVSP ≥35 mmHg abnormal. Its reference table lists ≤34 mmHg in the normal band. These are reference limits rather than a stand-alone diagnostic test. Interpret the unrounded estimate with the complete study and clinical context.
Clinical considerations
RVSP approximates pulmonary artery systolic pressure only in the absence of pulmonic stenosis or RV outflow tract obstruction. It is not mean pulmonary artery pressure.
Incomplete or misaligned TR envelopes and inaccurate RAP estimates can substantially distort RVSP. Severe tricuspid regurgitation may also undermine the pressure estimate.
Pulmonary hypertension is a hemodynamic diagnosis. An elevated RVSP supports further assessment in context; a low or unavailable estimate does not reliably exclude it. Right-heart catheterization may be needed for diagnosis and classification.
Common questions
What is the normal RVSP range?
The 2025 ASE reference table places RVSP ≤34 mmHg in its normal band and generally considers ≥35 mmHg abnormal. Reference limits must be interpreted with age, physiology, image quality, and other findings.
Does elevated RVSP mean pulmonary hypertension?
It can raise suspicion, but does not establish the diagnosis. RVSP is an echocardiographic estimate and can be affected by measurement error and outflow obstruction.
